Organization: Home of the Sparrow, Inc.
Location: McHenry Shelter (On-site)
Status: Full-Time, Monday – Friday, 7:00 – 3:00 pm

Position Summary
The Shelter Case Manager provides case management, client-centered consultation, and community support for residents at the McHenry Shelter. This role includes supporting clients’ goals, coordinating services, and ensuring a safe, welcoming shelter environment.
Key Responsibilities:
-Deliver weekly case management, individualized support, and client-centered consultation to assigned clients.
-Complete Individual Treatment Plans with clients every six months and ensure adherence to clients’ plans.
-Coordinate discharge planning and provide aftercare services upon client request.
-Maintain accurate client documentation, follow billing procedures, and uphold HOS guidelines for client savings accounts.
-Assist with client goal planning, including career, education, job readiness, and soft skill development.
-Conduct skill assessments, track employment program outcomes, help with resumes, cover letters, mock interviews, and determine eligibility for internships, vocational training, or certifications.
-Advocate on behalf of clients and connect them to appropriate resources.
-Provide transportation for clients as needed.
-Build positive relationships with the community and consistently represent Home of the Sparrow in a favorable light.
-Support shelter operations, including room preparation, cleaning, organizing binders for move-ins and move-outs, weekly shelter duties, and grocery trips.
-Adhere to specific shelter responsibilities as directed by the Shelter Manager (e.g., Child Care Volunteers, Tuesday/Thursday Night Programming, House Meetings, vehicle maintenance, chart reviews, etc.).
-Conduct rotating safety and maintenance inspections.
– Count petty cash daily and help ensure the shelter is clean, safe, and in good repair.
-Mentor and orient new shelter staff and volunteers as assigned.
– Attend and fully participate in all scheduled trainings and monthly supervision
meetings with the Shelter Manager.
-Collaborate with agency caseworkers and community partners to support client employment and overall case management.
Qualifications:
-Bachelor’s degree required.
-Previous experience working with high-barrier populations required.
-Valid driver’s license required.
-Strong written and verbal communication skills.
-Ability to work effectively with a diverse group of clients, staff, and community partners.
– Ability to work independently and as part of a team.
– Comfortable lifting up to 30 pounds and performing physical tasks including balancing, stooping, kneeling, crouching, crawling, reaching, sitting, standing, and walking for extended periods.
