Based at: Shelter
Status: Full-time & Part-time, Non-Exempt
General Purpose: The Case Manager will provide Case Management, Client-Centered Consultation, and Community Support to clients at the McHenry Shelter.
Key Responsibilities:
- Meets with the Shelter Manager at least monthly for supervision.
- Upholds Home of the Sparrow Rules, as defined by the Shelter Rules and Philosophy document.
- Follows emergency procedures as needed.
- Relates positively to the community, representing HOS in a positive manner at all times.
- Assists in preparing rooms, binders etc. for resident move-ins and move-outs as directed by the Shelter Manager.
- Attends and fully participates in all scheduled trainings as required by the agency.
- Complete Individual Treatment Plans with clients every six months.
- Follows clients’ Individual Plans carefully.
- Provides Case Management, Client-Centered Consultation, and Community Support Services to assigned clients weekly (and/or as needed). Involves other providers as needed, provides referrals and linkages, and provides discharge planning and coordination.
- Documents and bills all client interventions appropriately in client charts.
- Provides aftercare services upon client request.
- Upholds HOS guidelines for client savings accounts.
- Advocates on behalf of clients when indicated.
- Provides transportation for clients as it pertains to their Individual Plans as needed.
- Completes all weekly shelter duties as assigned by the Shelter Manager.
- Adheres to specific shelter responsibilities/assignments as directed by the Shelter Manager (i.e. Book Buddies, Child Care Volunteers, Tuesday and Thursday Night Programming, House Meetings, HOS vehicle maintenance, Chart Reviews, scheduled grocery shopping trips, etc.)
- Conducts rotating Safety and Maintenance Inspections.
- Counts Petty Cash daily.
- Helps ensure that the shelter is clean, safe and in good repair.
- Orientates new volunteers for shelter program nights.
- Orientate and mentor new shelter staff as assigned by Shelter Manager.
- Assesses shelter clients career goals and related experience, education,and work history.
- Utilize skill assessment tools.
- Collaborate with agency caseworkers.
- Track employment program outcomes.
- Determine a plan for the employment needs of clients.
- Help with resumes,cover letters,and mock interviewing.
- Be able to determine who is eligible for internship,vocational training, certifications, etc.
- Provide soft skill and job readiness services to candidates.
- Work collaboratively with our partners for employment opportunities,
- Maintain files according to program compliance requirements.

- Good written and verbal communication skills.

- Ability to work independently, as well as with a team.
Supervisory Responsibility: None
Physical Demands:
- Ability to lift up to 30 pounds.
- Balancing, stooping, kneeling, crouching, crawling and reaching are required.
- Sitting, standing and walking for extended periods of time.
Qualifications:
- Bachelor’s Degree Required.
- Previous experience working with high barrier populations Required.
- Must have a valid driver’s license.
