Status: Full-time, Non-Exempt
General Purpose: The Housing Coordinator is responsible for providing case management and referral services to clients participating in Home of the Sparrow housing programs.
Key Responsibilities:
- Provide case management, client-centered consultation, and community support services to Housing clients, involving other providers as needed.
- Manage a full-time caseload of 12-16 clients.
- Conduct criteria screenings and in-person interviews with prospective clients.
- Coordinate housing search: including calling landlords, driving around desired rental locations looking for possible apts., schedule showings with clients, inspect units to meet program and grant criteria for housing.
- Assist client with lease review and understanding.
- Prepare check requisitions for needed financial assistance with appropriate receipt documentation.
- Provide intensive case management resulting in housing attainment within 30 days of intake for homeless women and children.
- Develop and maintain positive relationships with area landlords.
- Meet with clients at least 2x monthly or more if needed, providing services such as budgeting, employment assistance, advocacy, life skills building and more.
- Complete Individual Treatment Plans and Biopsychosocial Assessments every six months.
- Manage wait list for Housing programs, including gathering the necessary documents to qualify for program.
- Provide referrals and linkages as needed to community partners, and provide discharge planning and coordination for Housing programs.
- Meet with Housing Manager for supervision at least monthly.
- Attend and fully participate in all Team meetings.
- Attend and fully participate in all scheduled trainings as required by Agency.
- Manage resident move-ins and move-outs and coordinate with Logistics Team.
- Document interventions in resident charts.
- Provide aftercare services when needed.
- Relate positively to the community, representing HOS in a positive manner at all times.
- Complete monthly and quarterly reports and outcome data as required.
- Participate in agency reviews and audits.
- Complete Monthly billing for Mental Health Board.
- Manage data reporting for HMIS system.
- May be required to work evenings or weekends to meet client needs.
Qualifications:
- Bachelor’s Degree required.
- Strong written and verbal communication skills.
- Previous experience working with high-barrier populations.
- Good problem solver and strong attention to details.
- Ability to work independently, as well as with a team.
Physical Demands:
- Ability to lift up to 30 pounds.
- Balancing, stooping, kneeling, crouching, crawling and reaching are required.
- Sitting, standing and walking for extended periods of time.

Pay: $20.00 – $23.00 per hour
