Job Title: Intake Navigator
Location: Woodstock, IL
Status: Part-Time
Join our mission-driven team at Home of the Sparrow, where we provide hope, opportunity, and support to women and children facing homelessness. We’re hiring a compassionate, organized Intake Navigator to coordinate our Homeless Prevention Program and serve as the first point of contact for individuals seeking shelter or support.
Job Purpose: The Intake Navigator is responsible for Coordinated Entry implementation, answering calls from community members in need of shelter or referral services and assessing their eligibility requirements for Home of the Sparrow Programs. The Intake Navigator is also responsible for attending outreach events as needed.
Key Responsibilities:
– Answer community phone calls and emails, and offer referral information as needed.
– Become an expert in community resources to offer informed referral to intake callers.
– Handle callers and walk-ins in crisis appropriately.
– Schedule and conduct phone and in-person interviews for potential clients.
– Complete screenings for prospective new clients for all Home of the Sparrow Programs and coordinate with appropriate case workers.
– Attend outreach events within the community as needed.
– Offer referrals and supportive services as needed throughout the intake process.
– Enter clients into HMIS and billing system and exit clients from systems at discharge in a timely manner.
– Track client data into the waitlist/Coordinated Entry document.
– Provide advocacy, emergency services, and referrals.
– Facilitate connection to local emergency shelters if appropriate.
– Outreach to community providers as necessary.
– Complete monthly reports, quarterly reports, and outcome data as required by funders.
– Ensure Intake procedure manual is current and followed.
– Document client interventions and billing entries in a timely manner.
– Participate in Intake Coordinator’s meeting at the Mental Health Board.
– Participate in Coordinated Entry committee meetings as needed for the Continuum of Care for McHenry County.
– Meet with Intake Manager for supervision at least monthly.
– Attend and fully participate in all Team Meetings.
– Attend and fully participate in all scheduled trainings required by the agency.
– Relate positively to the community, representing HOS in a positive manner at all times
Knowledge & Skills:
– Strong clinical skills preferred.
– Strong written and verbal communication skills.
– Previous experience working with high-barrier populations preferred.
– Good problem solver and strong attention to details.
– Bilingual preferred.
– Ability to work independently, as well as with a team.
– Prior knowledge of McHenry County community resources preferred.
Qualifications:
– Bachelor Degree in Social Work, Human Services or related field is preferred.
– 1-2 years of experience working with high barrier individuals is preferred.
– Has a valid Driver’s License.
Physical Demands:
– Ability to lift up to 30 pounds.
– Balancing, stooping, kneeling, crouching, crawling and reaching are required.
– Sitting, standing and walking for extended periods of time.
